简介:Thedamageofhumancornealcellsencounterwiththeproblemofavailabilityofcornealcellsforreplacement.Limitationofthesourceofcornealcellshasbeenrealized.Anattemptofdevelopmentofcornealepithelial-likecellsfromthehumanskin-derivedprecursor(hSKPs)hasbeenmadeinthisstudy.Combinationofthreeessentialgrowthfactors:epidermalgrowthfactor(EGF),keratinocytegrowthfactor(KGF)andhepatocytegrowthfactor(HGF)coulddemonstratesuccessfullyinductionofhSKPstodifferentiationintocornealcells.Theinducedcellsexpressedtheappearanceofmarkersofcornealepithelialcellsasshownbythepresenceofkeratin3(K3)byantibodylabelandWesternblotassay.TheK3geneexpressionofinducedhSKPscellsasshownbyreversetranscription-polymerasechainreaction(RT-PCR)technologywasalsodemonstrated.ThepresenceofthesemarkersatbothgeneandproteinlevelscouldleadtoourconclusionthatthedirectionaltransdifferentiationofhSKPscellsintocornealepithelialcellswassuccessfullydoneunderthiscellinductionprotocol.Thefindingshowsanewlyavailablestemcellsourcecanbeobtainedfromeasilyavailableskin.Cellsfromautologoushumanskinmightbeusedforcornealdisordertreatmentinfutureclinicalapplication.
简介:AIM:Toestablishanuntransfectedhumancornealstromal(HCS)celllineandcharacterizeitsbiocompatibilitytoacellularporcinecornealstroma(aPCS).·METHODS:PrimaryculturewasinitiatedwithapurepopulationofHCScellsinDMEM/F12media(pH7.2)containing20%fetalbovineserumandvariousnecessarygrowthfactors.Theestablishedcelllinewascharacterizedbygrowthproperty,chromosomeanalysis,tumorigenicityassay,expressionofmarkerproteinsandfunctionalproteins.Furthermore,thebiocompatibilityofHCScellswithaPCSwasexaminedthroughhistologicalandimmunocytochemistryanalysesandwithlight,electronmicroscopies.·RESULTS:HCScellsproliferatedtoconfluence2weekslaterinprimarycultureandhavebeensubculturedtopassage140sofar.AcontinuousuntransfectedHCScelllinewithapopulationdoublingtimeof41.44hoursatpassage80hasbeendetermined.Resultsofchromosomeanalysis,morphology,combinedwiththeresultsofexpressionofmarkerproteinandfunctionalproteinssuggestedthatthecellsretainedHCScellproperties.Furthermore,HCScellshavenotumorigenicity,andwithexcellentbiocompatibilitytoaPCS.·CONCLUSION:Anuntransfectedandnon-tumorigenicHCScelllinehasbeenestablished,andthecellsmaintainedpositiveexpressionofmarkerproteinsandfunctionalproteins.Thecellline,withexcellentbiocompatibilitytoaPCS,mightbeusedforinvitroreconstructionoftissue-engineeredHCS.
简介:AIM:Todemonstratethemorphologyandstructureofinvitroreconstructedtissue-engineeredhumancornealepithelium(TE-HCEP)withseedercellsfromanuntransfectedHCEPcellline.·METHODS:TheTE-HCEPswerereconstructedinvitrowithseedercellsfromanuntransfectedHCEPcellline,andscaffoldcarriersofdenudedamnioticmembrane(dAM)inair-liquidinterfaceculturefor3,5,7and9days,respectively.Thespecimenswereexaminedwithhematoxylin-eosin(HE)stainingofparaffin-section,immunocytochemicalstaining,scanningandtransmissionelectronmicroscopy.·RESULTS:DuringinvitroreconstructionofTE-HCEP,HCEPcellsformeda3-4,6-7and8-10layersofanHCEP-likestructureondAMsinair-liquidinterfaceculturefor3,5and7days,respectively.Butthecellsdeceasedto5-6layersandthestructureofstraifiedepitheliumbecamelooseatday9.Andthecellsmaintainedpositiveexpressionofmarkerproteins(keratin3andkeratin12),cell-junctionproteins(zonulaoccludens-1,E-cadherin,connexin43andintegrinβ1)andmembranetransportproteinofNa+-K+ATPase.TheHCEPcellsinTE-HCEPwererichinmicrovillionapicalsurfaceandestablishednumerouscell-cellandcell-dAMjunctionsatday5.·CONCLUSION:ThemorphologyandstructureofthereconstructedTE-HCEPweresimilartothoseofHCEPinvivo.TheHCEPcellsinthereconstructedTE-HCEPmaintainedthepropertiesofHCEPcells,includingabilitiesofformingintercellularandcell-extracellularmatrixjunctionsandabilitiesofperformingmembranetransportation.TheuntransfectedHCEPcellsanddAMscouldpromisinglybeusedinreconstructionHCEPequivalentforclinicalcornealepitheliumtransplantation.
简介:AIM:Toexaminethemechanismofthedevelopmentofpseudoexfoliation(PSX)syndromeviabothcytokineformationandendothelialvasorelaxingandgrowthfactorsthatwillprovideusnewtherapeuticinsightsforthetreatment.METHODS:Thisisacrosssectionalstudyincludedtwogroups;Group1:controlpatientswithnuclearcataract(n=20,aged51-80years).Group2:PSXpatientswithnuclearcataract(n=18,aged50-90years).Patientswithotherophthalmicproblemsandsystemicdiseaseswereexcluded.Vascularendothelialgrowthfactor(VEGF),interleukin-6(IL-6)andinterleukin-1β(IL-1β)andnitrotyrosinelevelsweredeterminedthroughserumsamplesbyEnzyme-linkedimmunosorbentassay(ELISA)method.Nitrite-nitratelevelsweremeasuredwithphotometricendpointdetermination.RESULTS:Therewerenosignificantdifferencesbetweenthegroupsintermsofage,VEGF,IL-1β,nitrite-nitrateandnitrotyrosine.ThesignificantresultswerethemeanIL-6levelsthatwerehigherinPSXgroup2(37.68±29.52pg/mL)comparedtothatincontrolgroup1(15.32±10.08pg/mL)(P<0.001).CONCLUSION:SeveralinteractingandextendingbiochemicalpathwaysmayleadtothepromotionofVEGFandIL-6expressions.IL-6whichistheonlyalteredmarkerinourstudymayindirectlycauseanincreaseofvascularpermeabilityandneovascularization.WesuggestinflammationasafactorthatcanbeinvolvedinetiopathogenesisofPSX.
简介:AIM:Toanalyzetheapplicationofmicrobubblecontrasttechnologyinthetreatmentofophthalmicdiseases,mainlyanalyzingitsadvantagesandexistingproblems.METHODS:Atotalof30representativeliteraturesabouttheapplicationofultrasoundcontrastagentingenetargetedtherapyathomeandabroadwerecollected,andfocusingonsortingouttheliteraturereportingthetreatmentofophthalmicdiseaseswithmicrobubblecontrasttechnologyinrecentyears,thenrecallingitsadvantagesandproblems,finallymakingreasonableassessmentonexistingproblemsandproposingpossiblesolutionstotheproblems.RESULTS:Duetoitsuniquesafetyandefficacy,thetreatmentofophthalmicdiseaseswithmicrobubblecontrasttechnologyhasincreasinglydrawntheattentionofclinicians,buttworelevantissuesshouldbeconsidered:first,thenatureofcontrastagentandthechoiceofcorrespondingultrasoundparameters;second,relativeincidenceoftissuebleeding,intravascularhemolysis,moderateorsevereallergyaswellasothersideeffects.CONCLUSION:Microbubblemaybecomethecarrieroftargetedtherapy,andasakindofnewnon-invasivedeliverysystem,theultrasoundcontrastagenthasbroadapplicationprospects,butitsapplicationinophthalmicresearchisstillinitsinitialstageandthesafetyofcontrast-enhancedultrasoundstillneedsfurtherstudy.

简介:AIM:ToevaluatetheeffectofCollagencross-linkingonthepreventionofmeltinginrabbitcorneasafteralkaliburn.·METHODS:TwentyNewZealandwhiterabbitswererandomlydividedintomodelcontrolgroupandcollagencross-linkingtreatmentgroup.Thesecondgroupofrabbitsreceivedcollagencrosslinkedtreatment.Bothgroupswereappliedwithantibioticeyedropstopreventinfection.Thecorneaswereevaluatedformelting,opacity,pathologicalandimmunohistochemistry,recordthechangeswhen28daysaftertheanimalswerekilled.·RESULTS:Inthecontrolgroup,6outof8rabbitsshowedcornealmeltingafterinjury(14±4)days,whiletwocornealperforated.Incollagencross-linkingtreatmentgroup,onerabbitshowedcornealmeltingafterinjury23days,withoutcornealperforation;cornealdissolutionratebetweenthetwogroupswassignificantlydifferent(P<0.05).Pathologicalexaminationsuggestedthatinthetreatmentgroup,mildcornealedema,milddamagetocollagenfibers,inflammatorycellinfiltrationwassignificantlylessthanthecontrolgroup.Immunohistochemistryshowedthatcornealcollagenfibersarrangedinneatrowsinthecontrolgroup.·CONCLUSION:Collagencross-linkingtreatmentnotonlycanpreventanddelaythecornealmeltingafteralkaliburn,butalsocanreducethedestructionofcornealcollagenfibersandinfiltrationofinflammatorycellsinthecornealtissue.
简介:AIM:Toevaluatetheeffectsandsafetyofphacoemu-lsification(Phaco)orsmall-incisionextracapsularcataractsurgery(SICS)andintraocularlens(IOL)implantationforagedpatients.METHODS:Totally137agedpatients(149eyes)underwentcataractoperationinthecaseofstablesystemiccondition,thebloodpressurelessthan160/95mmHg,bloodglucoselessthan8mmol/L,andunderthehelpofelectrocardiogramsurveillancebyanesthesiologistsduringtheoperation.106agedpatients(114eyes)underwentPhacowhile31agedpatients(35eyes)underwentSICS.Thepostoperativevisualacuity,cornealendothelialcellloss,surgerytimeandmajorcomplicationswereobservedandanalyzedretrospectively.RESULTS:Thebest-correctedvisualacuity(BCVA)of≥0.6wasachievedin135eyes(92.6%)at1monthpostoperatively(χ2=259.730,P<0.001).Foragedpatients,bothPhacoandSICScouldsignificantlyimprovevisualacuitywithnosignificantdifference(χ2=4.535,P>0.05).Postoperativecornealendothelialcelllosswas18.6%,inPHACOgroup,theratewas18.5%;inSICSgroup,theratewas19.0%,thedifferenceofwhichwasnosignificant(χ2=0.102,P>0.05).Thesurgerytimewasdifferentintwogroups.Noseverecomplicationsoccurred.CONCLUSION:BothPhacoandSICScombinedwithIOLimplantationforagedpatientsareeffectiveandsafe.Beforesurgery,detailedphysicalexaminationshouldbeperformed.Whenthesystemicconditionisstable,cataractsurgeryforagedpatientsissafe.
简介:目的探讨玻璃体的手术对复杂无晶状体眼视网膜脱离(AphakicRetinalDetachment,ARD)和人工晶状体眼视网膜脱离(PseudophakicRetinalDetachment,PPRD)(PVRC级以上)的治疗效果.方法回顾性分析78例87眼ARD和PPRD,所有病人均行玻璃体切割联合眼内填充物治疗.结果首次手术视网膜复位率为78.16%,最终视网膜复位率90.8%.视力下降11眼(12.7%),不变15眼(17.2%),提高59眼(70.1%).结论彻底的玻璃体切除加视网膜切开、视网膜前膜剥除,适当应用环扎玻璃体填充物可使复杂PPRD,ARD达到较高的视网膜复位率.但ARD、PPRD一旦发生则病情进展迅速,治疗棘手,视力预后差,应从白内障手术开始加强预防.
简介:AIM:Toassesstheeffectivenessofimmunosuppressantsintheprophylaxisofcornealallograftrejectionafterhigh-riskkeratoplastyandnormal-riskkeratoplasty.METHODS:WesearchedtheCochraneCentralRegisterofControlledTrials(CENTRAL),MEDLINE,EMBASE,CNKI,VIPandreferencelistsofarticles.Dateofmostrecentsearch:18June,2011.Allrandomisedcontrolledtrials(RCTs)assessingtheuseofimmunosupressantsinthepreventionofgraftrejection,irrespectiveofpublicationlanguage.Twoauthorsassessedtrialqualityandextracteddataindependently.Onlydichotomousoutcomes(cleargraftsurvival,ratioofimmunereactionsandsideeffects)wereavailableandwereexpressedasrelativerisk(RR)and95%confidenceintervals(CI).RESULTS:Sevenstudieswereincludedinthisreview.Inthecomparingofmycophenolatemofetil(MMF)withplacebo,theresultsshowedMMFcouldsignificantlyreduceimmunereactionscomparedwithplacebo(RR1.0895%Cl0.95to1.21),butnoeffectoncleargraftsurvival(RR1.1195%Cl0.90to1.35).Incleargraftsurvivalandimmunereactions,MMFandcyclosporineA(CsA)showedsimilareffect(RR1.1195%Cl0.90to1.35,andRR1.48,95%Cl0.56to3.93,respectively).Tacrolimus(FK506)andsteroidshowedsimilareffectsoncleargraftsurvivalandimmunereactions(RR0.32,95%CI0.02to6.21,andRR1.00,95%CI0.88to1.14,respectively).Nodrugrelativesideeffecthasbeenfound.CONCLUSION:MMFmayreduceimmunereactionsinbothnormal-riskandhigh-riskrejectionofpenetratingkeratoplasty.CsAandFK506showedsimilareffectsasMMF.However,duetothelackoflargeclinicaltrials,theevidenceremainweak,thequalityofevidenceswereratedasverylowtomoderate.Large,properlyrandomised,placebo-controlled,doublemaskedtrialsareneededtoevaluatetheeffectofimmunosuppressants.

简介:AIM:Toreportthelong-termvision-threateningcomplicationsinpatientswhounderwentphakicintraocularlens(pIOLs)implantationforhighmyopia.METHODS:Thisstudywasdesignedfromaconsecutiveseriesofphakicintraocularlenscomplicationandcorrectivesurgeries.Sixteeneyesof13patientshadimplantationofphakicintraocularlensforcorrectionhighmyopiaanddevelopedseriouscomplicationshavebeenincludedinthisstudy.Themeanageofpatientswas38.6±6.35y(range32-50y)andthemeantimeofhistoryofpIOLimplantationforhighmyopiawas6±2y(range2-10y).Beforecorrectivesurgery,bestspectaclecorrectivevisualacuity(BSCVA)rangedfromperceptionto20/200intheeyesinwhichseverecomplicationsoccurred.RESULTS:Cornealdecompensationoccurredin12eyesof9highmyopicpatientsafteranteriorchamberpIOLimplantation.Rhegmatogenousretinaldetachment(RRD)occurredin4eyesof4highmyopicpatientsfollowinganteriorchamberandposteriorchamberpIOLimplantation.Patientswithcornealdecompensation,hadcombinedproceduresconsistingofpIOLremovalandpenetratingkeratoplasty(PKP).RemovalsofpIOL,phacoemulsificationandparsplanavitrectomy(PPV)withsiliconeoiltamponadewereperformedinpatientswithRRD.Aftercorrectivesurgeries,allpatientsbutone(P+,patient2,righteye)achievedmoderateBSCVArangedfrom20/200to20/50atthelastvisit.CONCLUSION:PhakicIOLsmaybeeffectiveforthecorrectionofhighmyopia.AlthoughtheseIOLsmayhaveseverecomplicationsanditaffectssafetyandefficacyofthissurgery.Asseenhere,cornealdecompensationandrhegmatogenousretinaldetachmentarepossiblepostoperativevision-threateningcomplicationsofphakicIOLs.Patientsmustbecarefullyexaminedbeforeandaftersurgeryforpossibleendothelialcelllossandvitreoretinalproblems.
简介:AIM:Toassesstheawarenessofeyecomplicationsandtheprevalenceofretinopathy,inthefirstvisittoeyeclinic,amongtype2diabeticpatientsattendingatertiarymedicalcentreinKualaLumpur,Malaysia.METHODS:Aninvestigator-administeredquestionnairewasgivento137patientswithdiabetesundergoingfirsttimeeyescreeningintheeyeclinic.Thiswasfollowedbyadetailedfundusexaminationbyaseniorophthalmologisttoassessforpresenceofretinopathy.RESULTS:Almost86%ofrespondentswereawareofdiabeticeyecomplications,especiallyinpatientswhohadachievedtertiaryeducationallevel(96.3%).Themajorityofthepatients(78.8%)werereferredbytheirphysiciansandonly20.4%cameontheirowninitiative.Manyofthepatients(43.8%)didnotknowhowfrequenttheyshouldgoforaneyecheck-upand72.3%didnotknowwhattreatmentswereavailable.Lackofunderstandingondiabeticeyediseases(68.6%)wasthemainbarrierformostpatientsfornotcomingforeyescreeningearlier.Despiteahighlevelofawareness,only21.9%hadrecordedHbA1clevelof<6.5%while31.4%wereundertheerroneousassumptionofhavingagoodbloodsugarcontrol.Atotalof29.2%haddiabeticretinopathyintheirfirstvisiteyetesting.CONCLUSION:Inthepresentstudy,29.2%oftype2diabeticpatientshadretinopathyintheirfirsttimeeyetesting.Althoughtheawarenessofdiabeticeyecomplicationswashighamongfirsttimeeyescreeningpatients,theappropriateeyecare-seekingbehaviorwascomparativelylessandshouldberectifiedtopreventtheriseofthissightthreateningeyedisease.
简介:AIM:Tostudyclinicalfeaturesandgenemutationswithinthepaired-likehomeodomaintranscriptionfactor2(PITX2)geneinapedigreeofbilaterallimbaldermoids.METHODS:Completeeyeexaminationshavebeenperformedoneachindividualofthefamily.Exonsofpaired-likehomeodomaintranscriptionfactor2(PITX2)wereamplifiedbypolymerasechainreaction,sequenced,andcomparedwithareferencedatabase.RESULTS:Wedescribedthephenotype,clinicfindingsinafamilywithtwoaffectedmembers.Themassesoftheproband’seyeswereexcisedsurgicallydemonstratingadermoidcystbyhistopathologicalexamination.NomutationwasdetectedinthegenePITX2inthispedigree.CONCLUSION:Afamilyoflimbaldermoidcystwasreported.Inaddition,nopathogenicsequencevariationswerefoundinPITX2,indicatingthatthisphenotypeinthisfamilyisadistinctiveentity.

简介:AIM:ToidentifythefunctionofST2andexploretheroleofIL-33/ST2signalinginregulatingthepro-allergiccytokineproductioninhumancornealepithelialcells(HCECs).METHODS:HumancornealtissuesandculturedprimaryHCECsweretreatedwithIL-33indifferentconcentrationswithoutorwithdifferentinhibitorstoevaluatetheexpression,locationandsignalingpathwaysofST2inregulatingproductionofpro-allergiccytokineandchemokine.TheexpressionofmRNAwasdeterminedbyreversetranscriptionandrealtimePCR,andproteinproductionwasmeasuredbyenzyme-linkedimmunosorbentassay(ELISA),immunohistochemicalandimmunofluorescentstaining.ST2proteinwasdetectedindonorcornealepithelium,andST2signalwasenhancedbyexposuretoIL-33.·RESULTS:IL-33significantlystimulatedproductionofpro-allergiccytokinesthymicstromallymphopoietin(TSLP)andchemokine(CCL2,CCL20,CCL22)inHCECsatbothmRNAandproteinlevels.Thesestimulatedproductionsofpro-allergicmediatorsbyIL-33wereblockedbyST2antibodyorsolubleST2protein(P<0.05).Interestingly,theIκB-αinhibitorBAY11-7082orNF-κBactivationinhibitorquinazolineblockedNF-κBp65proteinnucleartranslocation,andalsosuppressedtheproductionsofthesepro-allergiccytokinesandchemokineinducedbyIL-33.CONCLUSION:ThesefindingsdemonstratethatIL-33/ST2signalingplaysanimportantroleinregulatingIL-33inducedpro-allergicresponses.IL-33andST2couldbecomenovelmoleculartargetsfortheinterventionofallergicdiseasesinocularsurface.
