简介:目的探讨血清特异性结核病抗体的诊断价值和对肺炎的鉴别诊断意义。方法采用酶联免疫吸附试验(ELISA)法对73例结核病,1054例肺炎及93例正常儿童进行血清特异性结核抗体检测,用PPD皮试作对照。结果结核组、肺炎纽及正常组的PPDlgG/IgM阳性率分别为65.75%、10.91%及3.22%。肺炎组与正常组比较X^2=5.47、P<0.05肺炎组符合儿童结核病诊断者17例、占1.01%占肺炎组结核抗体阳性病例的14.78%。ELISA法的敏感性为0.658特异性为0.968,阳性预测值为0.941,阴性预测值为0.782。结论检测血清PPPlgG/IgM有辅助结核诊断价值肺炎患儿的结核感染高于正常儿童,故应作为结核的重点防治对氛注意从中发现结核病患儿。
简介:麻疹被国家列为乙类法定传染病,具有高度的传染性,曾引起过世界大流行,是一种高发病率和高病死率的疾病。随着麻疹疫苗的推广使用,麻疹发病率和病死率大幅度下降,但麻疹仍呈散发状态,局部爆发时有发生,而且大多由症状典型转变为症状不典型的轻型感染或无症状隐性感染病例。随着麻疹流行的下降,许多临床医师未见过麻疹病人,也无诊断的经验。因此,依据临床表现作为麻疹诊断的单一标准
简介:目的检测梅毒特异抗体IgM,IgG,以了解梅毒感染的阶段,亦能观测抗梅毒治疗的效果。材料方法使用DiaSorinEIA试剂盒对69份梅毒TPHA阳性标本(2000/01-2000/06)进行梅毒特异性抗体IgG,IgM检测。结果IgG抗体阳性68例;其中IgM,IgG抗体均阳性9例;IgM抗体不确定1例;IgM,IgG抗体为阴性1例;IgM阳性IgG不确定1例。结论使用ELA方法可以直接检测抗梅毒特异抗体IgM,IgG,较传统RPR,TRUST,TPHA方法而言,能对抗体分型,能提示梅毒感染的各阶段,具有高特异性和高敏感性,对诊断、治疗更有益。
简介:目的评价抗苍白螺旋体IgM抗体检测对梅毒的临床意义。方法用酶联免疫吸附测定法对72例梅毒患者进行特异性IgM抗体检测,并与RPR、TPPA结果进行比较分析。结果血清抗苍白螺旋体IgM抗体在一期梅毒的阳性率为73.3%(11/15),在二期梅毒的阳性率为88.9%(16/18),二者比较无显著性差异(x~2=1.6363,P>0.10)。在潜伏梅毒,IgM抗体阳性率为26.1%(6/23),显著低于早期显性梅毒(x~2=17.6189,P<0.005)。在一期、二期和潜伏梅毒,RPR和TPPA的阳性率均为100%。入组前2-24个月己经给予正规抗梅治疗的梅毒16例,其中IgM抗体阳性2例。结论在本研究中,检测特异性IgM抗体诊断一期梅毒并下优于RPR和TPPA。IgM抗体在潜伏梅毒敏感性低,其诊断应依靠RPR和TPPA。目前不推荐单独检测抗梅毒IgM抗体来监测病情和判断疗效。抗苍白螺旋体IgM抗体的临床意义有待更深入的研究。

简介:Highlevelsoflowmolecularweight(LMW)IgMincertaindiseasesareassociatedwithclinicalandlaboratoryindiceswhichreflecttheseverityofthedisease.TheseassociationssuggestthatLMWIgMmayplayanimportantroleintheimmunopathogenesisofthesediseases.TofurtherapproachthequestionconcerningthefunctionalactivityofLMWIgMindisease,apanelofLMWIgMandhighmolecularweight(HMW)IgMpreparationswithorwithoutrheumatoidfactor(RF)activitywereusedtoinvestigatetheirantibodybindingactivityandtheireffectorfunction.ItwasfoundthatLMWIgM-RFandHMWIgM-RFhadasimilarbindingcapacitytoFcfragmentastherewasnosignificantdifferenceintheaffinityindexbetweenthem.ItfurthershowedthattherateofactivationandtotalamountofutilizationofcomplementbyLMWIgMandHMWIgMwassimilar,althoughthemeanfluorescenceofC3depositionbyIgM-RFandHMWIgM-RFwasslightlyhigherthanthatofLMWIgM-RFandothercontrolRFantibodies.However,thecurrentstudydemonstratedthatLMWIgMhadstrongneutrophilactivatingpropertieswhencomparedwithHMWIgM.ThesefindingssuggestthatonemechanismofLMWIgMcontributingtotheimmunopathogenesisofRAmaybeduetotheformationofcirculatingimmunecomplex(CIC)byLMWIgMwithsubsequentactivationofneutrophils.WhetherLMWIgMhasotherfunctionalactivityindiseaseisunclearandneedsfurtherinvestigation.
简介:目的探讨严重急性呼吸综合征患者血清SARS冠状病毒抗体IgM、IgG检测意义。方法应用ELISA法对16例SARS患者、147例发热隔离患者、23例体格检查健康者进行血清SARS抗体:lgM、IgG检测。结果12例SARS患者单份标本中入院标本7例有4例检出SARS抗体IgM阳性,出院5例有3例检出SARS抗体IgG阳性,2例重症患者血清动态结果观察到,从入院到出院的标本均同时检测到高滴度的SARSIgM、IgG;2例轻症SARS患者各只检测到一次SARS抗体IgM弱阳性;发热隔离区患者有6例从入院至出院均检测到SARS抗体IgG,其中两例滴度较高。健康体检者无一例SARS抗体阳性。结论ELISA检测SARS冠状病毒抗体IgM、IgG可作为一种辅助鉴定严重急性呼吸综合征患者的临床检测和确诊手段。
