简介:目的比较近视性弱视儿童与同龄矫正视力正常的儿童屈光状态变化的差别。方法收集2006年1月~2015年12月诊为近视性屈光不正的儿童96例(174眼),按初诊时屈光状态分为低、中、高度近视。分别比较近视性弱视眼组与同期矫正视力正常的非弱视近视眼组年均屈光度改变情况。结果非弱视组和弱视组近视屈光度每年加深(-0.67±0.74)D和(-1.09±1.38)D,差异有统计学意义(P〈0.05);初诊为低度、中度和高度近视的弱视眼近视屈光度年均加深(-0.97±0.88)D,(-1.48±1.29)D和(-1.89±1.06)D,低度、中度和高度近视的非弱视眼分别为(-0.57±0.61)D,(-0.68±0.49)D和(-0.78±0.29)D;低、中、高度近视性弱视儿童的近视屈光度年均加深幅度均显著高于非弱视眼(P〈0.05)。结论近视性弱视儿童与单纯近视儿童一样,近视屈光度随年龄增长逐年增加,近视程度愈高,增加的幅度愈大。且初诊为不同程度近视性弱视眼的近视屈光度逐年加深幅度均大于非弱视眼。
简介:目的探讨角膜绷带镜在原发性翼状胬肉切除联合结膜连续锁边缝合术后的应用效果。方法选取我院91例(91眼)原发性翼状胬肉患者,依据随机数字表法分组,均行翼状胬肉切除联合结膜连续锁边缝合术,对照组45例术中进行常规处理,观察组46例术后配戴角膜绷带镜,观察比较两组术后1d、5d、7d疼痛程度(VAS)评分,并统计角膜上皮愈合率及复发率。结果观察组术后1d、5d、7dVAS评分小于对照组,差异有统计学意义(t1=7.245,P1=0.000;t2=6.842,P3=0.000;t3=5.135,P3=0.000);观察组术后7d角膜上皮愈合率为97.83%(45/46),高于对照组80.00%(36/45),差异有22统计学意义(χ~2=5.680,P=0.017);观察组复发率为2.17%(1/46),低于对照组17.78%(8/45),差异有统计学意义(χ~2=4.587,P=0.032)。结论原发性翼状胬肉切除联合结膜锁边缝合术后配戴角膜绷带镜可促进角膜上皮愈合,减轻患者眼部疼痛程度,并降低复发率。
简介:目的探讨小梁切除术中连续锁边缝合的两种不同结膜瓣对泪膜稳定性的影响。方法双眼原发性青光眼患者26例,双眼均行小梁切除术,左眼组:以穹窿部为基底的结膜瓣组26例26眼;右眼组:以角膜缘为基底的结膜瓣组26例26眼,两组中结膜瓣均采用连续缝合密闭切口,并检测术前1d、术后1d、7d、14d、30d、60d的泪膜破裂时间(BreakupTime,BUT)、基础泪液分泌试验(SchirmerItest,SIT)、角膜荧光素染色积分(fluorescenceintegral,FI)。结果两组间BUT、SchirmerI试验于术后1d、7d、14d、30d及60d比较差异均有统计学意义(均P0.05)。结论连续锁边缝合的以穹窿为基底结膜瓣的小梁切除术显著降低了泪膜的稳定性,为提高患者的舒适度我们主张以角膜缘为基底作为手术切口。
简介:AIM:Todefinetheultrasonographicstructureofnormallowereyelidanatomiccompartmentsandtheirspacialrelationshipindynamicmotion.METHODS:Highresolutionultrasound(15MHz)wasperformedonthelowereyelidsof7normalsubjects.Movementsofthelowereyelidanditscompartmentswerevisualizedwithultrasound.Inaddition,themaximalexcursionareaofthelowereyelidfatcompartmentsandretractormotionswasmeasuredbeforeandaftermotion.RESULTS:Theorbicularismusclecouldbeseenasanecholucentstructurebetweenthedermisandtheechodencefatpads.Lowereyelidfatpadseemstobedividedinto2compartmentsasrangeofmotionanddirectionofmovementofeachofthemvaries.Itseemsthatthesecompartmentshavealsodifferentbehavior.Themeasuredprofileareaofthevisiblenormallowereyelidfatpadsduringmovementofglobefromup-gazetodown-gazedecreasedby50%.Orderofmovementoflowereyelidstructuresseemstobeasfollows:afterglobemovementfistweseeretractormovement,anteriororbitalfatpad,thenskinandseptum,andfinallymovementofinferiorfatpad.CONCLUSION:Ultrasoundrepresentsanoninvasivetoolforthevisualizationoflowereyelidmorphology.Expandingitsapplicationcouldhelpusunderstandthecompartmentalchangesinphysiologicaleyelidmovement,inaginganddiseasedstudypopulations,aswellasassessoperativeoutcomes.
