  • 简介:生物力学是研究组织或器官力量与功能之间关系学科,对研究眼科疾病预防、发生、发展、诊疗具有重要作用。眼球是一个封闭近似球体器官,眼球内外每一组织都有一定张力,而且相互联系,互为影响。眼内外及球壁某一局部组织改变,可能会影响眼球某一区域组织生物力学或功能改变。随着生物力学方法增多,大家越来越重视对眼部组织生物力学研究,达到预防和治疗眼部疾病目的。如使用胶原交联增强角膜强度,防治圆锥角膜或扩张性角膜疾病;又如后巩膜加固术增强巩膜力学方法来治疗或预防高度近视等。对眼外肌、角膜、巩膜、虹膜、晶状体生物力学研究是目前热点,本文就目前眼部生物力学研究热点做一综述。

  • 简介:AIM:Toassessthequantitativeassociationbetweenanisometropiamagnitude(AM)andthelossesofresolutionandcontrastsensitivity;andtoexemplifyhowthefunctionoffusionandstereopsisvarywithAMinpreviouslyuntreatedanisometropicamblyopia.METHODS:Atotalof57patientswithpreviouslyuntreatedanisometropicamblyopiawithoutstrabismus(range:8-35years),weremeasuredrefractiveerror,bestcorrectedvisualacuity(BCVA),fusionandstereopsis,and48patientshavecompletedcontrastsensitivityfunctiontest.AMwasdeterminedbydioptricvectoradditionmodel,andtheamblyopiadepthwasdeterminedbythedifferenceofBCVAinlogMARunitsbetweentheamblyopicandfelloweyes.RESULTS:AMwassignificantlycorrelatedwithbothamblyopiadepth(PearsonR=0.728,P<0.001)andtheinter-oculardifferenceoftheareaunderthelogcontrastsensitivityfunction(AULCSF)(R=0.505,P<0.001).Depthofamblyopiaandtheinter-oculardifferenceofAULCSFwasalsosignificantlycorrelated(R=0.761,P<0.001).Themoreseverityofamblyopia,thepoorerlevelsofcontrastsensitivity.MostpureanisometropeswithAMwaslessthan3.0Dretainfusionandsomestereopsis,butwhenAMweremorethan3.0D,especiallyfortheanisometropeswhoseAMwasmorethan6.0D,fusionandstereopsisfunctionwereseriouslyimpaired.CONCLUSION:Inthepatientswithpreviouslyuntreatedanisometropicamblyopia,higherdegreeofanisometropiaissignificantlyassociatedwithdeeperamblyopia,worsecontrastsensitivity,fusionandstereopsisfunctions.

  • 简介:目的比较面神经垂直段CT定位和解剖定位。方法15开颅取脑后颅底标本,对面神经管垂直段至外耳道后壁、外耳门后缘、颈静脉窝外侧壁与前壁水平之间距离进行直接测量,并与64层螺旋CT测量进行比较。结果尸骨直接测量与CT测量结果分别是(3.01±0.65)mm与(3.09±0.68)mm,(10.98±1.72)mm与(10.98±1.64)mm,(3.99±1.23)mm与(3.91±1.17)mm,(8.32±1.86)mm与(8.24±1.92)mm;两种方法测量值差异无统计学意义(P〉0.05)。结论CT测量方法可靠,测得值可信。CT扫描有助于明确诊断。

  • 简介:目的研究PBL教学模式眼科学教学中应用。方法60名学生分为两组,分别采用PBL教学法和传统教学法进行教学,比较两组学生对眼科学兴趣和考试成绩。结果PBL教学组学生对眼科学兴趣要显著高于传统教学组(P〈0.05),其考试成绩(82.00±9.06)也明显高于对照组(76.90±9.82)(P〈0.05)。结论PBL教学法能够提高学生主动性,增强学生学习兴趣和学习成绩,适于眼科教学中推广。

  • 简介:目的探讨心理护理眼科护理中应用.方法分别对3种眼病190位患者心理变化进行总结和分析,并对患者实施了积极有效心理护理及健康知识教育.结果患者以一种良好心态接受治疗和护理,并让患者对其所患疾病有了正确认识,情绪稳定.从而提高了治愈率,大大减少了并发症发生,患者对我们服务质量较满意.结论对患者进行有效心理护理可以建立良好护患关系,让患者积极主动配合医护人员治疗,早日康复.

  • 简介:目的探讨视网膜脱离患者健康教育形式。方法以视网膜脱离患者教育对象,通过护士对患者入院时、手术前、手术后、出院后四个不同时期进行有目的、有计划、系统性教育,使患者掌握健康知识,并使其行为向有利于健康方向发展。结果视网膜脱离患者术后视力恢复较满意,且治疗期间未发生并发症。结论健康教育应用对视网膜脱离患者康复起到了促进作用,提高了患者适应能力和生活质量,保证了手术成功率,同时和患者建立了良好护患关系。

  • 简介:目的探讨完善小梁切除术,减少术中、术后并发症途径.方法抗青光眼小梁切除术中应用前房穿刺术,观察手术效果.结果术后3眼发生浅前房、发生率4.5%,经治疗前房深度恢复正常.术后随访3~12,眼压控制10~17mmHg(1mmHg=0.133kpa).结论前房穿刺术小梁切除手术中应用,能有效地控制术后低眼压性浅前房发生,并有助于手术操作,提高手术成功率.

  • 简介:目的探讨前房穿刺急性青光眼中降低眼压应用。方法对我院收治35急性青光眼患者应用前房穿刺术。结果35急性光眼患者应用前房穿刺术,有31能快速降低眼压,无操作意外和并发症。结论前房穿刺术可以作为急性青光眼中降低眼压一种方法,避免或减少应用甘‘露醇全身毒副作用,降低医疗风险。

  • 简介:目的探讨临床护理路径斜视围手术期中应用效果。方法将68斜视患者按入院单双日将其分为对照组和观察组各34,比较两组患者遵医嘱依从性、满意度、住院时间、住院费用情况。结果观察组患者遵医嘱依从性和满意度明显提高,住院时间和住院费用明显减少,与对照组比较有统计学意义(P〈0.05)。结论斜视围手术期应用临床护理路径是一种科学、合理、高效、个性化护理模式,值得推广。

  • 简介:AIM:Todemonstratethemorphologyandstructureofinvitroreconstructedtissue-engineeredhumancornealepithelium(TE-HCEP)withseedercellsfromanuntransfectedHCEPcellline.·METHODS:TheTE-HCEPswerereconstructedinvitrowithseedercellsfromanuntransfectedHCEPcellline,andscaffoldcarriersofdenudedamnioticmembrane(dAM)inair-liquidinterfaceculturefor3,5,7and9days,respectively.Thespecimenswereexaminedwithhematoxylin-eosin(HE)stainingofparaffin-section,immunocytochemicalstaining,scanningandtransmissionelectronmicroscopy.·RESULTS:DuringinvitroreconstructionofTE-HCEP,HCEPcellsformeda3-4,6-7and8-10layersofanHCEP-likestructureondAMsinair-liquidinterfaceculturefor3,5and7days,respectively.Butthecellsdeceasedto5-6layersandthestructureofstraifiedepitheliumbecamelooseatday9.Andthecellsmaintainedpositiveexpressionofmarkerproteins(keratin3andkeratin12),cell-junctionproteins(zonulaoccludens-1,E-cadherin,connexin43andintegrinβ1)andmembranetransportproteinofNa+-K+ATPase.TheHCEPcellsinTE-HCEPwererichinmicrovillionapicalsurfaceandestablishednumerouscell-cellandcell-dAMjunctionsatday5.·CONCLUSION:ThemorphologyandstructureofthereconstructedTE-HCEPweresimilartothoseofHCEPinvivo.TheHCEPcellsinthereconstructedTE-HCEPmaintainedthepropertiesofHCEPcells,includingabilitiesofformingintercellularandcell-extracellularmatrixjunctionsandabilitiesofperformingmembranetransportation.TheuntransfectedHCEPcellsanddAMscouldpromisinglybeusedinreconstructionHCEPequivalentforclinicalcornealepitheliumtransplantation.

  • 简介:AIM:Tocharacterizetheclinicalfeatures,diagnosis,treatmentandprognosisofuveitisassociatedwithankylosingspondylitis(AS)inChinesepatients.·METHODS:TwohundredandthreepatientswithuveitisassociatedwithASfollowed-upintheThirdMilitaryMedicalUniversityDapingHospitalbetween2005and2010wereretrospectivelyevaluatedinthisstudy.Completeophthalmologicalexaminationswereevaluatedatbaselineandduringthefollow-upperiod.Thegender,age,follow-uptime,meanfrequencyofuveitisonset,andaccompanyingeyeexaminationfindings,history,demographicalparameterswerereviewed.Allthepatientspresentedcompleteclinicalandradiologic(sacroiliac,lumbar,dorsalandcervicalspine,knee,ankle,shoulder,hip,elbow)evaluation.HLA-B27typingwasalsosearched.·RESULTS:Therewere203patientsdiagnosedwithASassociateduveitis.AllshowedsacroiliacX-raychangesindicativeofAS.Therewere184maleand19femalepatients.Theaverageageofpatientswas35±12(range18-50).Meanfollow-upperiodwas2.4years(1-5years).Acuteanterioruveitiswasthemostcommontypeofuveitisinbothgenders.121eyespresentedunilateralinvolvement(55.2%),and92eyespresentedbilateralinvolvement(45.3%)withonsetalternately.22eyesoccurredhypopyon,16eyeswerefoundanteriorvitreouscells,7eyeswerenotedreactivemacularedemaorexudation,29eyespresentedposteriorsynechiaeofiris,and14eyespresentedcataract,9eyespresentedsecondaryglaucoma,2eyespresentedbendcornealdegenerationand1eyespresentedatrophyofeyeball.Atthefinalvisit,uveitiswaswellcontrolledinmostpatients.·CONCLUSION:ASassociatedwithuveitisinChinesepatientsmainlymanifestsasacuteanterioruveitis.AcombinationofcorticosteroidswithothermydriasisagentsiseffectiveformostASassociatedwithuveitispatients.Ingeneral,theprognosisisgoodinthesecases.

  • 简介:AIM:Tocomparetheeffectoftopicallyadministeredandsubconjunctivallyinjectedbevacizumabonexperimentalcornealneovascularizationinratsfortwoweeksaftertreatment.METHODS:Twenty-eightSprague-Dawleyratsweredividedintofourgroupsof7animals.Eachcornealcenterofrighteyewascauterizedwithsilver/potassiumnitratefor8s.Aftercornealburning,bevacizumab(12.5mg/mL)wastopicallyadministeredthreetimesperday(TBgroup)fortwoweeksorsubconjunctivallyinjectedondays2and4aftercauterization(0.02mL;SBgroup).Asnegativecontrols,ratsreceived0.9%salinetopicallythreetimesperday(TSgroup)orsubconjunctivallyondays2and4(0.02mL;SSgroup).Digitalphotographsofthecorneaweretaken1and2weeksaftertreatmentandanalyzedtodeterminetheareaofcorneacoveredbyneovascularizationasthepercentageofcornealneovascularization.RESULTS:Oneweekaftertreatment,thepercentageofcornealneovascularizationwassignificantlylowerintheTBandSBgroupsthanintheTSandSSgroups(allP<0.05).Twoweeksaftertreatment,thepercentageofcornealneovascularizationwassignificantlylowerintheTBgroupthanintheTSgroup(P<0.05).Inallgroups,thepercentageofneovascularizationwasdecreasingastimepassed(allP<0.05)CONCLUSION:Topicallyadministeredbevacizumabhaslongerstandinganti-angiogeniceffectthansubconjunctivallyinjectedbevacizumabincornealneovascularizationfollowingchemicalinjuryinrats.

  • 简介:Choroidalneovascularization(CNV)isanuncommoncomplicationassociatedwithamacularhole.Inthiscasereportofararecondition,wepresentapathologicmyopiapatientwithaco-existentmacularholeandchoroidalneovascularmembrane.ThepatientwastreatedwithphotodynamictherapyforCNV,andthenvitreoussurgeryfortheretinaldetachmentandmacularhole.Attheendof4yearsfollow-up,hervisualacuitywasimprovedto0.1whilethemacularholeremainedopen.OpticalcoherencetomographyisausefulinspectionmethodofthediagnosisofCNVandmacularhole.

  • 简介:AIM:Tocomparethetrabecularmeshwork(TM)andirisapoptosisoftreatedanduntreatedprimaryopenangleglaucoma(POAG)patients.METHODS:Eighttreatment-naive,newlydiagnosed(group1)and11medlcaiytreated(group2)patientswithPOAGwereincludedinthestudy.Eachpatientunderwentalimbus-basedtrabeculectomy.TheTMandperipheralirisspecimensweredissectedoutandweresnap-frozeninliquidnitrogenandstoredat-80tuntiltheywereassayed.ApoptosisineachgroupwasassesedbyTUNELmethod.RESULTS:Themeanpatientagewas60.6±5.8years(53-68years)vs58.9±8.9years(47-70years)ingroup1andgroup2(P=0.859).Themeantreatmenttimeingroup2was22.2±7.3months(12-34months).ApoptoticindexesinTMandirisweresignificantlyhigherinPOAGpatientsusingmedication(group2)comparedtotreatment-naivePOAGpatients(group1)(P=0.004,0.015;respectively).CONCLUSION:LongtermadministrationoftopicalantiglaucomamedicationscausesadditionaltoxiceffectsonTM.

  • 简介:AIM:Toassesstheawarenessofeyecomplicationsandtheprevalenceofretinopathy,inthefirstvisittoeyeclinic,amongtype2diabeticpatientsattendingatertiarymedicalcentreinKualaLumpur,Malaysia.METHODS:Aninvestigator-administeredquestionnairewasgivento137patientswithdiabetesundergoingfirsttimeeyescreeningintheeyeclinic.Thiswasfollowedbyadetailedfundusexaminationbyaseniorophthalmologisttoassessforpresenceofretinopathy.RESULTS:Almost86%ofrespondentswereawareofdiabeticeyecomplications,especiallyinpatientswhohadachievedtertiaryeducationallevel(96.3%).Themajorityofthepatients(78.8%)werereferredbytheirphysiciansandonly20.4%cameontheirowninitiative.Manyofthepatients(43.8%)didnotknowhowfrequenttheyshouldgoforaneyecheck-upand72.3%didnotknowwhattreatmentswereavailable.Lackofunderstandingondiabeticeyediseases(68.6%)wasthemainbarrierformostpatientsfornotcomingforeyescreeningearlier.Despiteahighlevelofawareness,only21.9%hadrecordedHbA1clevelof<6.5%while31.4%wereundertheerroneousassumptionofhavingagoodbloodsugarcontrol.Atotalof29.2%haddiabeticretinopathyintheirfirstvisiteyetesting.CONCLUSION:Inthepresentstudy,29.2%oftype2diabeticpatientshadretinopathyintheirfirsttimeeyetesting.Althoughtheawarenessofdiabeticeyecomplicationswashighamongfirsttimeeyescreeningpatients,theappropriateeyecare-seekingbehaviorwascomparativelylessandshouldberectifiedtopreventtheriseofthissightthreateningeyedisease.

  • 简介:AIM:Toinvestigatewhethertheresponseofacentralhexagonalelementcorrespondingtothemacularareainconventionalmultifocalelectroretinography(mfERG)testswasthesameasthatofexperimentalmfERGusingsinglecentralhexagonalelementstimulation.METHODS:Prospective,observationalstudy.Thirtyhealthysubjectswereincludedinthisstudy.mfERGrecordingswereperformedaccordingtotwoprotocols:stimuluswith37hexagonalelements(protocol1),andstimuluswithasinglecentralelementcreatedbydeactivatingtheother36hexagonalelements(protocol2).Wecompareddifferencesbetweenring1parametersineachprotocol.RESULTS:Inprotocol1,thefirstpositivecomponent(P1)implicittimeandP1amplitudewere37.8±1.8msand6.3±2.7μV.Aftersingleelementstimulation(protocol2),doublepositivewavesappeared.TheimplicittimeandamplitudeofP1were40.7±2.4ms(P<0.001)and9.1±3.3μV(P=0.001),respectively.Theimplicittimeandamplitudeofthesecondpositivecomponent(P2)were68.0±4.5ms(P<0.001,comparedwithP1inprotocol1)and12.3±4.7μV(P<0.001,comparedwithP1inprotocol1),respectively.TheamplitudeofP2inprotocol2wasabouttwotimeshigherthanthatofP1inprotocol1.CONCLUSION:mfERGresponsesofacentralhexagonalelementinasingleelementstimulationprotocolaredifferentfromthoseofmultipleelementstimulation.Thepositivewaveismoreenhancedcomparedtothatoftheconventionalprotocolanditelongatedintotwowavelets.
