简介:AbstractAntibody-drug conjugates (ADCs) combine the high specificity of monoclonal antibodies with the high anti-tumor activity of small molecular cytotoxic payloads. The anti-tumor activity of ADCs is mainly achieved by the direct blocking of the receptor by monoclonal antibodies, direct action and bystander effect of cytotoxic drugs, and antibody-dependent cell-mediated cytotoxicity and complement-dependent cytotoxicity. ADCs have been used in adjuvant therapy and rescue treatment of human epidermal receptor 2 (HER2)-positive breast cancer, greatly improving the prognosis of breast cancer patients. Several ongoing clinical trials of ADC for breast cancer and other solid tumors proved the potential of ADCs will provide more promising treatment options for patients with malignant tumors. This review introduces the mechanism and latest clinical progress of ADC drugs approved for HER2-positive breast cancer to guide clinical practice and conduct research.

简介:Objective:Apreviousstudydemonstratedthatnon-anthracycline-containingdocetaxelpluscyclophosphamide(TC)regimenwasinferiortodocetaxel,anthracyclineandcyclophosphamide(TAC)inneoadjuvanttreatmentoftriple-negativebreastcancer(TNBC)andhumanepidermalgrowthfactorreceptor-2-(HER2)-positivebreastcancerinashort-termfollow-up.Herein,long-termfollow-upsurvivaloutcomeshavebeeninvestigated.Methods:TNBCorHER2-positivepatientswererandomizedtoreceive6cyclesofTCorTACneoadjuvanttreatment.Theprimaryendpointwaspathologicalcompleteremission(pCR).Secondaryendpointsincludedclinicalresponserate,event-freesurvival(EFS),andoverallsurvival(OS).Results:Acohortof96patientsconsistedof45inTCand51inTACarm.Withamedianfollow-upperiodof53(range,8–76)months,thepatientsachievingpCRpostneoadjuvantchemotherapyexhibitedsuperiorEFSandOSthanpatientswithoutpCR(P<0.05).TACtreatmentresultedinconsistentlybetterEFSthanTCtreatment:theestimated5-yearEFSwas66.1%vs.29.8%(P=0.002).Moreover,theestimated5-yearOSwasalsoinfavorofTAC:88.4%vs.51.6%(P<0.001).Multivariableanalysisdemonstratedthatthetreatmentregimenwasanindependentprognosticfactor,andpatientstreatedwithTAChadasuperiorEFS[hazardratio(HR),0.48;95%confidenceinterval(95%CI),0.26–0.90;P=0.021]andOS(HR,0.20;95%CI,0.08–0.60;P=0.003).Conclusions:Theupdatedlong-termfollow-updatademonstratedasustainedbenefitinEFSandOSfromanthracycline-containingTACtreatment,indicatingthatanthracyclineisanessentialandeffectivedruginthisclinicaltrial.

简介:摘要人表皮生长因子受体2(HER2)低表达乳腺癌具有较为独特的生物学行为,与HER2零表达乳腺癌相比,在分子分型与基因表达方面存在显著差异。以T-DXd为代表的新型抗体药物偶联物为HER2低表达乳腺癌患者提供了新的治疗选择,相关临床试验在持续开展中。目前,HER2低表达与乳腺癌患者预后的关系尚存争议。对HER2低表达乳腺癌的深入研究将有助于进一步改善该类乳腺癌患者的临床结局。

简介:目的:研究胃癌组织中TROP2和HER2蛋白表达,探讨TROP2和HER2蛋白在胃癌中的临床病理学意义以及二者的相关性。方法:应用免疫组织化学方法SP法检测56例胃癌组织中TROP2和HER2蛋白表达。结果:胃癌组织中TROP2和HER2蛋白阳性表达率分别为69.6%和32.1%。胃癌组织中TROP2蛋白表达与与淋巴结转移、淋巴血管侵犯、浸润深度及临床分期有关(P﹤0.05),而与患者性别、年龄、肿瘤部位、Lauren分型等无关(P﹥0.05)。HER2蛋白在胃癌中的表达与与Lauren分型、淋巴结转移和临床分期有关(P﹤0.05),而与患者性别、年龄、肿瘤部位、浸润深度、淋巴血管侵犯等无关(P﹥0.05)。胃癌中TROP2与HER2蛋白表达无相关性(P﹥0.05)。结论:TROP2高表达与胃癌的侵袭转移有关,提示检测TROP2可预测胃癌的临床生物学行为,可作为未来靶向治疗的一个靶点。
简介:目的:探讨人表皮生长因子受体2(humanepidermalgrowthfactorreceptor2,HER2)和环氧化酶2(cyclooxygenase-2,COX-2)在非小细胞肺癌(non-smallcelllungcancer,NSCLC)中的表达,其各自与临床特征之间的关系及二者在NSCLC中表达的相互关系。方法采用免疫组化链霉菌抗生物素蛋白-过氧化物酶(streptavidin-peroxidase,SP)法检测72例NSCLC中HER2、COX-2的表达情况。结果①72例NSCLC中,HER2、COX-2的阳性表达率分别为55.6%、65.3%;②HER2在NSCLC中的阳性表达与组织学类型、分化程度、肿块大小、淋巴结转移及临床分期相关(P<0.05),而与性别、年龄、吸烟、病变部位无关(P>0.05);③COX-2的阳性表达与年龄、组织学类型、肿块大小、淋巴结转移及临床分期相关(P<0.05),而与性别、吸烟、病变部位及分化程度无关(P>0.05);④HER2、COX-2在NSCLC中的阳性表达呈正相关关系。结论HER2、COX-2可以作为判断NSCLC恶性程度的生物学指标。
