简介:AIM:Toevaluatetheefficacyandsafetyofcornealcollagencrosslinking(CXL)topreventtheprogressionofpost-laserinsitukeratomileusis(LASIK)cornealectasia.·METHODS:Inaprospective,nonrandomized,single-centrestudy,CXLwasperformedin20eyesof11patientswhohadLASIKformyopicastigmatismandsubsequentlydevelopedkeratectasia.Theprocedureincludedinstillationof0.1%riboflavin-20%dextranesolution30minutesbeforeUVAirradiationandevery5minutesforanadditional30minutesduringirradiation.Theeyeswereevaluatedpreoperativelyandat1-,3-,6-,and12-monthintervals.Thecompleteophthalmologicexaminationcompriseduncorrectedvisualacuity,bestspectacle-correctedvisualacuity,endothelialcellcount,ultrasoundpachymetry,cornealtopography,andinvivoconfocalmicroscopy.·RESULTS:CXLappearedtostabiliseorpartiallyreversetheprogressionofpost-LASIKcornealectasiawithoutapparentcomplicationinourcohort.UCVAandBCVAimprovementswerestatisticallysignificant(P<0.05)beyond12monthsaftersurgery(improvementof0.07and0.13logMARat1year,respectively).Meanbaselineflattestmeridiankeratometryandmeansteepestmeridiankeratometryreduction(improvementof2.00and1.50diopters(D),respectively)werestatisticallysignificant(P<0.05)at12monthspostoperatively.At1yearafterCXL,meanendothelialcellcountdidnotdeteriorate.Meanthinnestcorneapachymetryincreasedsignificantly.·CONCLUSION:Theresultsofthestudyshowedalong-termstabilityofpost-LASIKcornealectasiaaftercrosslinkingwithoutrelevantsideeffects.Itseemstobeasafeandpromisingproceduretostoptheprogressionofpost-LASIKkeratectasia,therebyavoidingordelayingkeratoplasty.

简介:Choroidalneovascularization(CNV)isanuncommoncomplicationassociatedwithamacularhole.Inthiscasereportofararecondition,wepresentapathologicmyopiapatientwithaco-existentmacularholeandchoroidalneovascularmembrane.ThepatientwastreatedwithphotodynamictherapyforCNV,andthenvitreoussurgeryfortheretinaldetachmentandmacularhole.Attheendof4yearsfollow-up,hervisualacuitywasimprovedto0.1whilethemacularholeremainedopen.OpticalcoherencetomographyisausefulinspectionmethodofthediagnosisofCNVandmacularhole.
简介:AIM:Tocomparethetrabecularmeshwork(TM)andirisapoptosisoftreatedanduntreatedprimaryopenangleglaucoma(POAG)patients.METHODS:Eighttreatment-naive,newlydiagnosed(group1)and11medlcaiytreated(group2)patientswithPOAGwereincludedinthestudy.Eachpatientunderwentalimbus-basedtrabeculectomy.TheTMandperipheralirisspecimensweredissectedoutandweresnap-frozeninliquidnitrogenandstoredat-80tuntiltheywereassayed.ApoptosisineachgroupwasassesedbyTUNELmethod.RESULTS:Themeanpatientagewas60.6±5.8years(53-68years)vs58.9±8.9years(47-70years)ingroup1andgroup2(P=0.859).Themeantreatmenttimeingroup2was22.2±7.3months(12-34months).ApoptoticindexesinTMandirisweresignificantlyhigherinPOAGpatientsusingmedication(group2)comparedtotreatment-naivePOAGpatients(group1)(P=0.004,0.015;respectively).CONCLUSION:LongtermadministrationoftopicalantiglaucomamedicationscausesadditionaltoxiceffectsonTM.
简介:AIM:Toinvestigatetheantifibroticeffectofthefreeze-driedbilayeredfibrin-bindingamnioticmembraneasadrugdeliverysystemonglaucomasurgeryinrabbitmodel.Theaimofthisstudywastoprepareanovellocaldeliverysystemforthesustainedandcontrollablereleaseof5-Fu.METHODS:Twenty-fourJapanesewhiterabbitswererandomizedintothreegroups:theexperimentalgroup(oculartrabeculectomyincombinationwith5-Fuloadedfreeze-driedbilayeredfibrin-bindingamnioticmembranetransplantation),thecontrolgroup(oculartrabeculectomyincombinationwith5-Fu)andtheblankgroup(singletrabeculectomy).HEstaining,massionstainingandimmunohistochemistryforα-SMAwereperformedondays7,14,21and30followingsurgery.Theconcentrationof5-Fuinrabbitaqueoushumorwasexaminedbyhighperformanceliquidchromatography(HPLC)3daysafterthesurgery.RESULTS:Statisticaldifferenceswerenotedinintraocularpressureamonggroupsonday7,14,21and30followingsurgery.Histologyfurtherdemonstratedthattrabeculectomyincombinationwithfreeze-driedbilayeredfibrin-bindingamnioticmembraneyieldedwellwoundhealingandnoscarformationandwasbeneficialforlongtermeffect.CONCLUSION:HPLCshowedagoodslow-releaseeffectwithfreeze-driedbilayeredfibrin-bindingamnioticmembrane.
简介:AIM:Todeterminethehistopathologicalchangesofrifampicinappliedintravitreallyonretinalganglioncellsbymeansofstereologicalandhistopathologicalmethods.METHODS:Forthisstudytwenty-fourNewZealandadultrabbitsweredividedintofourgroups(n=6foreachgroup).50μg/0.1mL(group1),100μg/0.1mL(group2),150μg/0.1mL(group3)and200μg/0.1mL(group4),rifampicinwereinjectedintothevitreousoftherighteyesofanimals,theirlefteyeswereusedascontrol(group5).Afterthe28thdayofapplication,animalswereanesthetisedwithxylazine(8mg/kg,IM)andthentheireyeswereenucleatedimmediately.Patternsweretakenawayandeyeswerepreparedforbothstereologicalandelectromicroscopicobservation.RESULTS:Dependingonthehighdoseofrifampicin,somehistopathologicalchangessuchascytoplasmicdilatationanddamagedmembranewereobservedontheelectromicroscopiclevel.Usingquantitativeexamination,whichwasdoneatthelightmicroscopiclevel,itwasshownthatthenumberofneuronsdecreasedlinearlyasrifampicindoseincreasedwhencomparedwiththecontrolgroup.CONCLUSION:Basedonthesefindings,low-doserifampicin(50μg/0.1mL)maybeusefulfortreatmentoftheoculardiseases.
简介:摘要目的分析在服用卡马西平后引起的重症药疹的治疗方案以及临床效果。方法从我院在2011年5月至2015年7月收治的238例因药疹住院的患者中选取16例服用卡马西平引起的重症药疹的患者,收集16例患者的临床资料,并对其进行分析。结果在这16例患者中,重症多形红斑有6人,大疱性表皮松解症有8例,剥脱性皮炎型有2例;卡马西平重症药疹的病势较严重,且病情发展较快,采用糖皮质激素治疗效果较好,这16例患者均治愈出院。结论卡马西平重症药疹会导致多器官功能紊乱,且发病急、病情重、发展快、病程长,在临床上大多采用糖皮质激素治疗,合理运用糖皮质激素,才是治疗成功与否的关键,在临床上应该慎用卡马西平。

简介:AIM:Totestthehypothesisthatamblyopicneuroretinamayhaveanalteredthicknesswhencomparedtothenormal.·METHODS:Twenty-fiveamblyopic,youngpatientsbetweentheagesof7and11yearsoldwerestudied.Theinterestedneuroretinaareasaredefinedinto10sub-regionsaccordingtosuperior-inferior,nasal-temoral,andperi-paraaxis,whichcrossthefovelastructure.Thethicknessesoften,definedmacularregionswereseparatelymeasuredbyopticalcoherencetomography(OCT)andanalyzedbyt-test.·RESULTS:Theaveragethicknessofneuroretinaintheexactfoveolaoftheamblyopiceyesislargerthanthatofnormaleyes(P<0.05),buttheothernineregionshavenosignificantdifference.Interestingly,inboththenormalandamblyopiceyes,thetemporalarealooksthinnerthanotherquadrants(P<0.05).·CONCLUSION:Thicknessalterationmaybeassociatedwithamblyopicdisordersinyoungpatients.Studyingalargervolumeofsubjectsofsimilarageisrequiredtoconfirmthisobservation.
