摘要
【摘要】 目的 研究分析全身麻醉联合硬膜外麻醉和全身麻醉在高血压老年患者腹腔镜胆囊切除术中的麻醉效果。方法 此次研究的对象是选择 100例行腹腔镜胆囊切除术的高血压老年患者，将其临床资料进行回顾性分析，并随机分成观察组和对照组， 各 50例。观察组给予全身麻醉联合硬膜外麻醉， 对照组给予全身麻醉。对比两组患者的麻醉效果。结果 两组患者麻醉过程中， 在麻醉前（ T1）测得其收缩压（ SBP）、舒张压（ DBP）、心率（ HR）比较， 差异无统计学意义（ P>0.05）。在气管插管后（ T2）、气腹后 10 min （ T3）、手术结束气管插管拔管前（ T4）时两组患者 SBP、 DBP、 HR明显高于 T1， 且观察组患者 SBP、 DBP、 HR数值明显小于对照组， 差异有统计学意义（ P<0.05）。观察组患者停用静脉麻醉药物后至意识恢复的平均时间明显小于对照组（ P<0.05）。结论 全身麻醉联合硬膜外麻醉在老年高血压患者行腹腔镜胆囊切除术麻醉过程中， 可有效的维持患者血压及呼吸系统的稳定， 术中麻醉效果好， 术后麻醉恢复时间快， 具有较好的临床应用效果， 值得临床推广应用。

[Abstract] Objective To study and analyze the anesthetic effect of general anesthesia combined with epidural anesthesia and general anesthesia in laparoscopic cholecystectomy for elderly patients with hypertension. Methods 100 elderly patients with hypertension who underwent laparoscopic cholecystectomy were selected and their clinical data were retrospectively analyzed. They were randomly divided into observation group and control group, 50 cases in each group. The observation group was given general anesthesia combined with epidural anesthesia, while the control group was given general anesthesia. The anesthetic effect of the two groups was compared. Results There was no significant difference in systolic blood pressure (SBP), diastolic blood pressure (DBP) and heart rate (HR) between the two groups before anesthesia (T1) (P > 0.05). SBP, DBP and HR in two groups were significantly higher than those in T1 after tracheal intubation (T2), 10 minutes after pneumoperitoneum (T3) and before tracheal intubation and extubation (T4), and the values of SBP, DBP and HR in observation group were significantly lower than those in control group (P < 0.05). The average time from withdrawal of intravenous anesthetics to recovery of consciousness in the observation group was significantly shorter than that in the control group (P < 0.05). Conclusion General anesthesia combined with epidural anesthesia can effectively maintain the stability of blood pressure and respiratory system in elderly hypertensive patients undergoing laparoscopic cholecystectomy. The anesthesia effect during operation is good, and the recovery time of anesthesia after operation is fast. It has good clinical application effect and is worthy of clinical popularization and application.
